Family Video Movie Review Contest Official Rules and Regulations
1) Description of Contest: The 2009 Family Video Movie Review Contest is sponsored by FamilyVideo.com, 2500 Lehigh Avenue, Glenview, IL, 60026. Sponsor will award one (1) first place prize, one (1) second place prize, one (1) third place prize, one (1) fourth place prize and one (1) fifth place prize and ten (10) finalist prizes to the finalists who do not place in the top 5. The five winners will be chosen by judges from the top 15 finalists.
2) Promotional Period: The Family Video Movie Review Contest promotional period begins Nov 13th 2009 at 12:00:00 am Central Time ('CDT') and ends January 7th, 2010 at 11:59:59 pm Central Time ('CDT'). All entries must be received during this time period to be considered as possible finalists. Judging will take place the week of January 10th-17th, 2010.
3) How to Enter: Upload your video to the official "Family Video Movie Review Contest Site". A valid email address will need to be entered when you upload yoru video. Duplicated entries will be voided. Family Video reserves the right to keep only the email addresses of persons registering for use in future promotions. By uploading your video to the Family Video site, you agree to allow Family Video to show your video for promotional purposes if it is chosen. In the unlikely event Family Video encounters technical failures, equipment failures, server and/or provider difficulties, or human errors in conjunction with the Family Video Movie Review Contest, Family Video will not be held responsible.
4) Eligibility: NO PURCHASE NECESSARY TO WIN. The contest is open to all legal residents of the 50 United States and the District of Columbia who are 18 years of age or with the permission. Void where prohibited or restricted by law. To be eligible to win, all entries must be received by Family Video in the format stated in Step 3 above and during the time period stated in Step 2 above. By submitting an entry to the contest, entrant agrees to abide by the Official Rules and Regulations and the decisions of Family Video, which are both final and binding.
5) Prizes and Descriptions: Fifteen (15) Prizes will be awarded to fifteen (15) eligible winners. One (1) first place winner will receive $1,000. One (1) second place winner will receive $500. One (1) third place winner will receive $250. One (1) fourth place winner will receive $150. One (1) fifth place winner will receive $100. Ten (10) finalists who do not place in the top five will receive a $25.00 FamilyVideo.com gift card.
6) Awarding of All Contest Prizes: A panel of judges will choose the 5 winners from a list of 15 finalists. The finalists and winners will be notified by e-mail. Failure to comply with Official Terms and Conditions, failure to complete and return required documents within fifteen (15) days of attempted delivery will result in awarding the prize to an alternate eligible winner. Failure of alternate eligible winner to comply with these conditions will result in a forfeit of prize and no additional eligible alternate winner shall be chosen. All eligible winners are responsible for taxes where applicable. By accepting the prize, the winner agrees that Family Video employees, managers, representatives, and all other affiliates with Family Video have no liability for, and shall hold all included parties harmless against damage, loss or injury that my arise, directly or indirectly, from the acceptance or use of prizes and/or participation in the Contest. Offer is void where prohibited.
7) The Winner's List: To see a list of winners of the contest, mail a self-addressed, stamped envelope to Family Video Corporate Headquarters, Family Video Movie Review Contest, 2500 Lehigh Avenue, Glenview, IL, 60026. |
